Directions for Filing a Claim for Unpaid Wages:
If you are claiming pay for work performed, please print out, fill out, and submit your form to the following address: Division of Labor and Employment Law, Virginia Department of Labor and Industry, North Run Business Park, 1570 East Parham Road, Richmond, Virginia 23228.
Remember to sign the claim
form and to make sure you include the employer's full address, as well
as your best estimate of the total wages owed to you. Incomplete forms
will be returned. After you submit the form, please notify the Department
if there is any change in the status of the claim, if, for example, you
receive payment, change your address or telephone number or file your
own claim in court.

Description: In Virginia all minors under
the age of 16 are required to have a work permit before starting to work.
Minors employed in dramas, plays, performances, concerts or other entertainment
are not required to have the regular work permit, but instead are required
by §§ 40.1-101 and -102 of the Code of Virginia to obtain a
theatrical permit from DOLI. The normal hours of work limitations applicable
to a regular work permit are not applicable to minors engaged in these
activities.
Instructions: Applications must be filed at least five
days before the performance date. They must be submitted to the Department
of Labor and Industry, Labor and Employment Law Division, 13 South Thirteenth
Street, Richmond, Virginia, 23219, or may be faxed to (804) 371-2324.

Solicitation Permits

Description: Section 40.1-112 of the Code of Virginia requires
all employers engaged in street trades in any public place, including
candy sales and solicitation for subscription contracts for books, magazines
or other publications (newspapers are excepted), to obtain a solicitation
permit from the Department of Labor and Industry. Nonprofit organizations
as defined in 26 U.S.C. § 501(c)(3) of the United States Internal
Revenue Code are exempt from this requirement. Separate permits are required
for each geographical area in which the employer plans to operate. Application
for the permit must be made no more than 30 days before the date the employer
plans to conduct business. The permits are valid from the date of issuance
to June 30th of the next year.
