The Occupational Safety and Health Laws of Virginia give employees the right to file complaints about workplace safety and health hazards. Further, the Occupational Safety and Health Laws give complainants the right to request that their names not be revealed to their employers. Complaints from employees and their representatives are taken very seriously by VOSH. Please do not use this form to file claims for unpaid wages. Instructions and forms to file claims for unpaid wages can be found in the Labor and Employment Law section of this site.

If you would like to report hazards at your worksite to VOSH, choose one of the following:

• Download the VOSH complaint form in PDF, complete it and then fax or mail it to the closest VOSH Regional or Field Office to you. Be sure to include your name, address and telephone number so we can contact you. Download FREE Adobe Acrobat Reader to view and print the VOSH complaint form in PDF. (Note: After completing the form using this format, please print it. You will not be able to save your work.)

• Download the VOSH complaint form in MS Word Template format, complete it and then fax or mail it to your local VOSH Regional or Field Office. Be sure to include your name, address and telephone number so we can contact you.

• Telephone the closest VOSH Regional or Field Office to you. VOSH staff can discuss your complaint and respond to any questions you have.
